Job Description:
A company is looking for a Remote Licensed Practical Nurse (LPN)
or Licensed Vocational Nurse (LVN) who is bilingual in Spanish and
English.
Key Responsibilities:
Manage patient census to close gaps in clinical and non-clinical
care
Identify and coordinate community resources for patient care
Provide education on chronic condition management and perform
medication management
Required Qualifications and Education:
Active Multi-State/Compact License (LPN) or (LVN)
At least 3 years of direct patient-facing work experience
Technical aptitude with Microsoft Office Suite
Bilingual in Spanish and English
Experience with chronic care management
